PDFCreator has a COM interface

You can install PDFCreator (a PDF printer) and then control the process using COM. After creating a new PDFCreator object, you'll basically set all the settings required for an "autosave" printing, and then you use the Print method of NotesUIDocument to print. After printing, you grab the PDF file and attach it or do whatever you want with it.

I set up a Domino server to start a Notes client to perform PDF conversions on documents queued to be mailed. It won't convert 100 documents a minute, but it does run reliably.
